بعدما صار يُنظر إلى أساقفة الكنائس الكبرى (رؤساء الأساقفة) في المدن الكبرى باعتبارهم أعلى مرتبة من أساقفة الكنائس الصغيرة، وبمرور الوقت أصبح يُنظر لكنائس خمس مدن باعتبارها ذات أهمية خاصة للغاية. وهذه المدن هي أورشليم، وأنطاكية، والإسكندرية، والقسطنطينية، وروما؛ وأصبح أساقفة كنائسها يسمون بطاركة.
